Job Title: Care Management Support Coordinator II
Contract Type / Duration: 6 months (possibility to extend or convert)
Location: Remote – Missouri only
Work Hours: M–Th 10:30 AM–7:00 PM CST; F 9:30 AM–6:00 PM CST (includes 30-min lunch and two 15-min breaks)
Pay Rate: $19/hr
Job Summary
The Care Management Support Coordinator II provides administrative support for care management activities. This role includes performing outreach, answering inbound calls, scheduling services, and serving as a primary point of contact for members, providers, and staff. The coordinator ensures accurate documentation of member records and supports onboarding, ongoing education, and referrals to community or health plan resources.
